Telehealth Appointments

We are exclusively offering TELEHEALTH APPOINTMENTS

now for clients who live anywhere in the United States.

You are now able to attend your appointment

from the comfort of your home without the need for traveling.

Patients are loving the multitude of conveniences with telehealth! Some schedule appointments while at work without needing to take vacation days. Some attend from home in their pajamas and comfy slippers.  All save money at the gas pump and time with travel.  The health care is exactly the same whether you are at home, at work, or sitting at our desk. 

Telehealth
is the use of electronic information and communication technologies by a provider to deliver health services to a client via a secure telehealth system (such as HIPAA-compliant Zoom which we use) comprised of both audio and video elements when the client is located at a different site than the practitioner. 

Dr. Bazylewicz now offers holistic health with telehealth for clients located throughout the United States.

Appointments will be scheduled on most weekdays mornings. No appointments are scheduled on weekends.

Usually we see patients initially, at one month, then two months later, and then at least every 2-3 months while on active protocols.  If your health is considerably compromised, then monthly follow-up consultations are recommended. 

Alternatively, you may choose just one appointment for a specific issue or you can just schedule annual wellness checks, etc.  In other words, we can design appointments around your wants and needs.

Telehealth Procedure

We have stream-lined the process to make it easy for everyone and to keep costs as low as possible.

 NEW PATIENT PROCEDURE


1.   REQUEST AN APPOINTMENT HERE 

2.   EMAIL PAPERWORK WHEN YOU REQUEST AN APPOINTMENT

  • If the appointment is for your minor child, then ALL parents/guardians must sign the paperwork.
  • Email to thehealthypath @ gmail.com (no spaces) as an attachment. 

3.  SEND PAYMENT WHEN YOU REQUEST AN APPOINTMENT

  • See the Make a Payment page to make a payment.
  • You will receive an electronic receipt after making payment.

4.  CONFIRM APPOINTMENT

  • You will receive an email after you complete the above three steps notifying you about possible appointment dates and times.  
  • Please let us know which date and time works best for you.  
  • You will then receive another email with your zoom link.

3.  SEND PREVIOUS LABS AND TESTS

  • Mail (if many pages) or email (if few pages) to 5706 Wilder Road, Grimsley, TN 38565 or thehealthypath @ gmail.com (no spaces), respectively. 
  • They should arrive at least 48 hours prior to your appointment so we can better prepare for your appointment.  (If they do not arrive in time, then we will discuss them at your next consultation - not a problem.)
  • Please do not send previous office notes. 
  • Please do not mail certified or insured or signature or any other way that requires us to visit the PO in order to avoid a $125 pick-up fee.

4.  ATTEND ZOOM APPOINTMENT

  • Use the unique Zoom link that you were sent in your reminder email to enter the waiting room prior to your scheduled appointment. 
  • Check that your audio and video are working.
  • Dr. Bazylewicz will start the call at your appointment time. 
  • If you experience problems at the time of your appointment, email thehealthypath @ gmail.com (no spaces) or call 937-444-4385.
  • After the call, Dr. Bazylewicz completes and emails the health plan to you.





 FOLLOW-UP CLIENT PROCEDURE


1.   CONFIRM YOUR APPOINTMENT BY RESPONDING TO YOUR REMINDER EMAIL.

2.  EMAIL PAPERWORK AT LEAST 48 HRS PRIOR TO YOUR APPOINTMENT

  • Email to thehealthypath @ gmail.com (no spaces) as an attachment.
  • They must arrive at least 48 hours prior to your appointment.
  • These forms are needed even if "nothing has changed" or if you are "not taking anything."

2.   SEND LABS AND TESTS IF APPLICABLE

  • Most labs ordered by us will come directly to us. However, if you have labs and tests that were done by another provider or hospital, or if we do not get the results, then follow these procedures.
